1、多組學(xué)綜合圖譜解析轉(zhuǎn)移性肝癌的時(shí)空演變

摘要:目前肝癌轉(zhuǎn)移腫瘤的基因突變和分子表達(dá)全景圖譜仍極度缺乏,肝癌在轉(zhuǎn)移過程中的克隆選擇等演化規(guī)律尚不清晰,導(dǎo)致肝癌轉(zhuǎn)移病灶的臨床診療仍然缺乏足夠的理論指導(dǎo)。在這里,作者對(duì)182例HCC患者的257個(gè)原發(fā)性和176個(gè)轉(zhuǎn)移灶進(jìn)行了多組分析。原發(fā)性和轉(zhuǎn)移性HCC的基因組差異很高,早期播散很普遍。在轉(zhuǎn)移瘤中觀察到的顯著的腫瘤內(nèi)新抗原異質(zhì)性與T細(xì)胞反應(yīng)性降低有關(guān),這是由于新抗原遞呈中斷所致。沒有Wnt突變的亞克隆比有Wnt突變的亞克隆顯示出更強(qiáng)的轉(zhuǎn)移選擇性優(yōu)勢,并以微環(huán)境中豐富的活化成纖維細(xì)胞為特征,有利于前轉(zhuǎn)移表型。最后,沒有Wnt突變的轉(zhuǎn)移瘤表現(xiàn)出更高的免疫抑制B細(xì)胞富集,通過HLA-E:CD94-NKG2A檢查點(diǎn)軸介導(dǎo)CD8+ T細(xì)胞的末端衰竭??偟膩碚f,作者的結(jié)果提供了一個(gè)復(fù)雜的轉(zhuǎn)移進(jìn)化過程的多維解剖。
